Share your photos with the MED13L Foundation
Thank you for sharing your photos and videos with our community. Seeing our families is one of the most powerful ways we spread awareness of MED13L Syndrome.
This form lets you send us your photos and records your consent for how we may use them. Sharing is completely voluntary. You choose exactly where your photos may appear, and you can withdraw your consent at any time, free of charge, by emailing social@med13l.org. Saying no, or withdrawing later, will never affect your family’s participation in Foundation programs, events, or research.
This is the International Media Release, for families who live outside the United States, including the EU and UK. If you live in the U.S., please use the U.S. residents release form.
Please read this before you begin. When we publish a photo, the people in it may be recognized as part of the MED13L community, and others may understand that they are living with MED13L Syndrome. Because of this, we treat your photos as sensitive information and only use them with your explicit consent.
Who we are. The MED13L Foundation, Inc., 14 Main Street, Park Ridge, IL 60068, United States, is responsible for your information. Your photos and this form are stored on Google servers in the United States. Contact social@med13l.org with any question about your information, or to ask for a copy of it, a correction, or its deletion. You may also complain to your local data protection authority.
