By purchasing a ticket to or attending the MED13L Family & Research Conference hosted by The MED13L Foundation, Inc. (the “Foundation”), you (the “Participant,” or if under 18, the legal guardian or parent of the Participant) agree to the following terms and conditions:
Health and Safety Acknowledgment
I understand that participation in the conference may involve exposure to potential risks, including illness. I acknowledge the importance of monitoring my health and agree not to attend if I experience symptoms of illness. I recognize that some participants may have medical vulnerabilities and will act accordingly to prioritize community well-being.
Release of Liability
I voluntarily assume all risks associated with participation and agree to release and hold harmless the Foundation, its directors, officers, volunteers, and agents from any claims or demands arising from my participation. This includes, but is not limited to, claims related to personal injury, illness, or property damage.
Separate Research Consent
I understand that any participation in research activities during the conference will require separate informed consent. I am not obligated to participate in any research, and my decision to participate or decline will not affect my ability to attend other conference activities.
Photo and Media Release
I grant the Foundation and CTNNB1 Connect and Cure Inc., the right to use any photographs, images, video, or audio recordings taken during the conference for promotional and educational purposes. If I do not wish to be photographed or recorded, I will make every effort to avoid being included in photos and will notify any photographers or conference organizers of my preference.Governing Law and Venue
This Waiver and Release shall be governed by and construed in accordance with the laws of the State of Illinois. Any legal action arising from this agreement shall be filed exclusively in the state or federal courts located in Chicago, Illinois. Both parties consent to the personal jurisdiction of these courts and waive any objection to venue. If any provision of this agreement is deemed invalid or unenforceable, the remaining provisions shall remain in full effect.
